Review the RFA for instructions on the letter of intent (LOI) as well as the full application. Use the form and template below to prepare your LOI and application.
Please note: Though not binding, a letter of intent (LOI) is required. Applications submitted without prior LOI will not be reviewed.
Letter of Intent (LOI)
The LOI for Cycle 7 was due on November 12, 2025. Please stay tuned for the release of the request for applications for Cycle 8.

Download and complete the Application & Data Request Form (in fillable MS Word).
Research Abstract & Narrative Template
Download the template (in MS Word) and follow the instructions within to prepare your abstract and research narrative. You may delete the instructions portions when the content is ready for submission.
Data Use & Transfer Agreement Template
Coming soon: A new sample DUA will be available on the iPHD website in December 2025.
Although not required at the time of application, the Data Use Agreement provides details on data security and privacy requirements for all applications approved by the iPHD Governing Board.
The Data Use Agreement, along with IRB documentations, will be required upon approval of project application and prior to data transfer.
